Copy of Latest Required Immunization Records - (If the well
visit for your child is after registration, please bring
updated
immunization
records to main office as soon as completed)

to school from home for transportation form
GRADES 1- 8

Baptismal Certificate - Original and Copy
Penance Certificate -
Original and Copy (If applicable)
Communion Certificate -
Original and Copy (If applicable)
Confirmation Certificate -
Original and Copy (If applicable)
Copy of Latest Required Immunization Records -
Students entering 6th Grade will need 1 Tdap or Adacel
and 1 meningococcal
Copies of all Report Cards, Standardized Tests,
and Child Study Team Reports for our school files
***Mileage from your home to St. Mary School for transportation form
Registration will not be confirmed until students
spend two full days at St. Mary School. This enables us
to determine if we are the school to meet your child’s learning needs.
For the student, it enables them to get to know the school and students a
little better and not wonder about this new step all summer.
*After the above
experience and a review of all records, we will confirm registration and request the registration fee at that
time.
Registration
is also dependent upon the availability of space.
***For both groups: Please have
determined beforehand, the mileage from
your home to St. Mary School
for the State Transportation forms.

P2 (3 year olds) must be 3 by October 1st.

All preschool students are required to have a flu shot
*After we confirm registration for the 4 year old
program we will request the registration fee at that time.
