Items Needed for Registration (For Students New to SMS) PRESCHOOL…                                             P1 (4 year olds) must be 4 by October 1st.                                    P2 (3 year olds) must be 3 by October 1st.                                     *Registration Fee - $100.00 (Non-Refundable)                                      Birth Certificate                                      Baptismal Certificate                                      Immunization Records:                                      DTP or DtaP - 4 doses                                      Hib – 1 dose after one year old                                      Polio - 3 doses                                      MMR vaccine – 1 dose after one year old                                      Varivax – 1 dose after one year old                                      Hepatitis B vaccine – Series of three – Initial dose, 2nd dose                                      no later than 12 months after the first dose, 3rd dose no                                      later than 12 months after the first dose                                      Influenza - 1 dose annually                                      Pneumococcal - 2 doses between the ages of 2-11 months,                                      1 dose after first birthday                 *After we confirm registration for the 4 year old program we will request the registration fee at that time.
